HOW TO BECOME A PATIENT

1. Call to make an appointment (415) 890-4449. If you plan to use your dental insurance benefits, be sure to have your insurance information available, including insurance company, group number and member ID.
   Or, book online.
   

2. Download and fill out the new patient forms.

   • New Patient Health History Form

   • Communication, Privacy and Policies Form

   • HIPAA and Privacy Policy

   • For children/minors:

     • Pediatric New Patient Health History Form

     • Pediatric Communication, Privacy and Policies Form
       

3. Come see us at 930 16th St. (bring your forms or email them to us in advance!)
